I inherited a sperm whale tooth and am considering making a shaving brush out of it. I know the golden nib has knots, but I don't have any experience with their quality. Does anyone know if there is a better place to get a superb silver tip or Manchurian knot?

Once you decide what size knot you want, look at Declaration Groomings web site to determine hole dimensions, then use their knotting service for the B14 knot.
Looks like the best vendors for quality knots have been listed. be sure to open your wallet wide. 
But my favorite for the past 6 months or so has been the Maggard SHD fan-shaped 24mm. For 46 bucks and the quality of the knot you can't go wrong IMO.

Wow Shavemac has some pricey options, seems legit though. Where does one order badger hair for creating their own knot?

Some folks just harvest from another brush and install that one into the desired handle. You will destroy the brush handle with the harvested knot but it can be beneficial also for cost and quality.
I haven't done one in a few years, but when I was restoring them regularly I used TGN knots.
My every day brush is probably about eight years old, and it is still going strong.
Edit; I just checked, and I actually put this together in October 2011, and has been my go to brush ever since. TGN Finest knot.

Shavemac knots are top, top shelf but not cheap.
I bought two super high density (SHD) 2 band badger knots from Maggard a couple of years ago. One is fantastic. The other one less so - the coloring does not look natural to my eyes. The Maggard SHD knots are truly SHD - no idea how they pack so much hair into the knot. A SHD 24 mm seems to have as much hair as a normal 26 mm.

Most of the others, even the ones mentioned here, are really third party retailers who get their knots from China of lesser quality, gelled (to make lesser quality bristles appear of higher quality), bleached (mostly deprived of the bristle natural oils), and simply resell them to make profit.

VanBrushes are selling some nice brushes and knots.
