Headphones?

I use Koss Portapros. Great in the gym and when I go for a run. They offer great sound quality and the are very affordable. I believe Costco has them online, if not the jungle site will have em
 
JVC Gummy plus (with remote and mic) work well for me. $15, they stay in, and can be used with my android phone for hands free.

I personally won't spend more on ear buds, as I use ear buds for walking, mowing the lawn, at work (factory/machines) etc. So far these JVC have been a quality product!

Edit (because its important): The sound quality is very good for the price. Bass is even not bad! I used to have $45 "noise cancelling" earbuds (don't remember brand...nothing cheapie though) that could not touch these for quality.

I started out using Sony MDR-V55's ($56 on amazon) but they kept slipping off every time I hit supine bench and any other workout where I wasn't upright. I switched to some yurbuds inspires (on sale for $22.50 on the yurbuds website) that I had laying around. They seem to be the only in ear headphone that will fit properly for me. The sound is decent even at higher volumes and it gets loud enough to drown out the gym noise.

Yurbuds, Ironman series. You cannot shake them out of your ears if you tried. I tried, and failed. Watch their video. Hilarious!
